What the enquiry form collects

If you send a message from the contact page, we receive what you type:

your name and phone number
your email address, if you choose to give it
what you're interested in, and the best time to reach you
your message, if you write one

We use this only to reply to you and set up a consultation.

Where your message goes

The form is delivered by a third-party form service, which emails your message to Katherine Naylor, who handles consultations for CP Birth Center.

Please don't send medical information

The form isn't meant for health information. Please don't include symptoms, diagnoses or medications in your message. Joanne will go over those with you privately at your consultation. You can also call or text Katherine at 435-212-3206.

Analytics

We count visits with a cookieless analytics tool, so we can see which pages are useful. It sets no cookies and doesn't identify you. We don't use advertising trackers or social media pixels.
